Dia Physio Care may offer limited-time promotional services or packages. These promotions:

  • Cannot be combined with any other offers or discounts
  • Do not guarantee insurance coverage or reimbursement
  • Are subject to appointment availability

Prepayment Requirement: all promotional offers require full prepayment at the time of booking. A valid credit card must be placed on file to secure the appointment. By booking a promotional service, you authorize Dia Physio Care to charge the full promotional amount to your credit card.

Please note that submitting your information or payment does not guarantee an appointment time until your booking has been confirmed by our clinic.

3. Insurance Billing

We offer direct billing with most major insurance providers as a courtesy. It is your responsibility to confirm your individual insurance coverage, limits, and eligibility prior to receiving services. Dia Physio Care is not responsible for denied or unpaid insurance claims.

4. Cancellations & No-Shows

We require at least 24 hours’ notice to cancel or reschedule an appointment. Late cancellations or no-shows may result in forfeiture of prepaid promotional fees or may be subject to an additional cancellation fee, at the clinic’s discretion.

5. Liability Disclaimer

While we strive to provide effective, evidence-based care, treatment outcomes may vary between individuals. Dia Physio Care and its staff are not liable for specific treatment results, complications, or delays in care.
